There is a medical prescription attachment box here on which customer can either upload attachment now for the order or upload the attachment later. There is no attachment later functionality for guest users, they need to upload the prescription at the time of placing order only.

Change of Prescription Status from Backend – WooCommerce Medical Prescription Attachment
Admin can view the attached medical prescription on the edit order page in the metabox from which prescription status gets changed and the respective email will be sent to the customer to inform regarding the approval or rejection. If prescription got rejected and if option is enabled from the configuration then customer can re-upload the prescription to the order in order to get approved.

When customer uploaded the medical prescription in the order then the mail with attachments will get delivered to the admin.
